Kim Kardashian is open about her desire to have another child, but admits the pregnancy wouldn't come without complications, including potential surgery beforehand.
In a new clip from Keeping Up With The Kardashians, the 36-year-old socialite tells her family that she may have to go under the knife.
"I have to go in and repair my uterus because I decided I'm going to try to have one more baby," she announces at the dinner table.
"Isn't that exciting?"
The look on Kris Jenner's face says it all, with the whole family appearing completely stunned.
"Oh my god, I thought you were for sure done [having kids]," Kendall then says.
"What surgery are we having?" sister Khloe then asks.
Kim explains: "I have to have a surgery on my uterus to kind of repair this hole, so they need to like clean that out and then there's scar tissue.
"It will still be a really high-risk pregnancy, just you'd be able to get pregnant."
At this point, all of the family members look slightly concerned as they remain silent, but Kimmy is determined to stick to her guns.
"Having more kids is definitely going to be a struggle," she admits in a piece-to-camera.
"I've gone through so much with really bad deliveries that the doctors don't feel like it's safe for me to conceive again myself.
"This surgery is really the one last thing I can try."
Kim already shares two children, daughter North, three, and one-year-old son Saint with husband Kanye West.
There have been health complications for the star before, who had preeclampsia during her pregnancy with North and placenta accreta while in labour with both kids.
She hasn’t been shy about sharing the complications she had with her previous pregnancies and took to her website in 2015 to write about her labour fears.
"Last pregnancy, I had a condition called preeclampsia, which is a serious condition you can get during pregnancy," she wrote.
"It causes your body and face to swell, and that was very uncomfortable for me.
“I had early-onset preeclampsia and I had to deliver at 34 weeks, almost six weeks early."
Kim went on to graphically describe the issues she had while giving birth to North.
"My placenta stayed attached inside my uterus, which is a condition called placenta accreta," Kim said.
"This is a high-risk condition that happens when the placenta grows too deeply into the uterine wall."
"My doctor had to stick his entire arm in me and detach the placenta with his hand, scraping it away from my uterus with his fingernail.
"My mom was crying; she had never seen anything like this before.
"They say that this is what some women died from as a result of childbirth back in the day, without proper care.
And while she was worried she might never conceive again, the reality star welcomed Saint West into the world in December 2015.
However his arrival wasn’t without its own complications, with Kim suffering from the same placenta issues, as well as a painful procedure to turn the breach baby around.
